Our Physical Therapist will be a professional that provides care as needed for our patients in a home setting.

You will support the needs of patients and work with other professionals to manage patients suffering from CHF/COPD or in need of anticoagulation therapy due to total hip/knee replacement.

Average visits require the PT to assess patient, provide teaching, manage risks, and work with other disciplines to improve patient outcomes.
