Description:
Safe Choices Nurse (RN): Providing specialty nursing services to help establish a successful
transition from hospital to home.

Visit various patients daily in their home for approximately 25-45 minutes each. Visits require
nurse to perform assessments, provide teaching, and manage transition processes, especially
medication management.

Interact with various disciplines to ensure patient outcomes are met. Team members include
Geriatric Care Manager (MSW or LCSW), Certified Nurses Aides, Physical Therapists, and
Occupational Therapist.

* RN licensed to practice professional nursing in Texas
* 3 + years experience preferred
* Experience in home care and/or geriatric populations preferred but not required
* Good people skills and ability to communicate with various levels of people
* Ability to work in varied environments
* Ability to self-manage with follow through to complete tasks
* Ability to work with others as well
* Detail oriented with ability to assess facts and draw conclusions
* Good with systems and procedures
* A good comfort level with computers is a plus.
