Job: MDS/CARE PLAN COORDINATOR - Must be LICENSED LVN/RN in the State of Texas

The MDS/CARE PLAN COORDINATOR is responsible for the timely completion and
transmission of the TEXAS MDS and completion of the Care Plan for each resident. RUG
Classification Knowledge a must, MDS Certification preferred. MUST BE A LICENSED LVN
or RN IN THE STATE OF TEXAS.

Job duties include but not limited to:

1.Assist other professional staff in developing and implementing policies for resident care.
2. In collaboration with other disciplines assures that each resident's comprehensive
assessment, care plan and discharge planning is maintained.
3. Maintains resident rights, including the right to privacy, confidentialty and dignity.
4.Performs other related duties as directed by the Director of Nursing.
5. Must be able to complete MDS's.
6. Must be computer literate.
7. Willing to attend specialized training as necessary
8. Attends and participates in all required facility meetings

Must be a Registered or Licensed Nurse who is currently licensed to practice by the State of
Texas.

Long term care experience in a skilled nursing facility a must!

Education and/or experience in nursing services administration preferred.

Education, training and/or experience in restorative or geriatric nursing preferred.

Must be able to read, write, speak and understand English.

Must be able to supervise and instruct others.
