U.S. Renal Care, Inc. (“USRC" or the “Company") which was founded in 2000, acquires, develops and operates dialysis centers in joint venture partnership with nephrologists in their local markets across the United States. The company’s clinics provide in-center hemodialysis and at home hemodialysis and peritoneal dialysis services related to end stage renal disease (“ESRD"). In addition, the company also manages several acute setting dialysis programs in conjunction with local community hospitals.

We currently have a career opportunity for you to join our North Texas clinics as a Licensed Master Social Worker.

Position Summary:
The Licensed Master Social Worker provides social services to patients and their families in order to promote maximum social functioning and psychological adjustment to dialysis and rehabilitation of the patient.

Responsibilities:
• Advocacy on patient’s behalf with the appropriate local, state, and federal agencies
• Provide patient and family education
• Provide information and referrals as needed
• Facilitate with community agencies referrals
• Case documentation
• Coordinate transient arrangements
• Coordinate transportation services
• Assist and verify information on patient’s monthly travel claims forms
• Provide updates on the monthly reports-Clinic’s patient census, Medicaid transportation list, and individual provider list
• Attend quality improvement and patient care plan meetings
• Initiate ESRD medical form and collaborate with physicians for completion of form
• Complete death notification form
• Complete initial assessment of new patient psychosocial needs in adherence to state guidelines
• Counsel patients and families relative to identified psychosocial issues or problems
• Identify social agencies and other resources as appropriate to the needs of the patients, including but not limited to financial/funding sources. Provide monthly report to Regional Administrator regarding patient financial issues/status using designated report format
• Coordinate development and maintenance of patient/family support groups
• Collaborate with the team to establish a care plan and long term plan with revisions as needed
• Comply with organization policies and procedures
Be familiar with professional organizations related to kidney disease (i.e. National Kidney Foundation, American Kidney Foundation)

Skills / Requirements
• Master’s Degree in Social Work accredited by CSWE required
• Current licensure in the state of Texas (LMSW/LCSW) required
• Previous experience in the care of dialysis patients preferred
• Demonstrate a working knowledge of the English language and be able to communicate verbally and in writing
