Job: Case Manager, Tranisitonal Living Program

Summary: Provides case management through evaluating a thorough needs assessment, providing legal advocacy, and victim assistance to adult clients in the Transitional Living program.


Major Responsibilities: (Other duties may be assigned)

Deliver crisis intervention services to all shelter residents.

Provide case management through a thorough needs assessment, legal advocacy, and victim assistance to adult client of the emergency shelter.

Facilitate face –to-face sessions with residents to assist in the development of individualized case management plans and to ensure personal safety and attainable exit goals.

Complete a outcome measure tool to evaluate the client’s functioning in areas such as but not limited to personal responsibility, social networks, living skills, physical health, awareness of safety concerns, exit options, etc.

Assist with client transportation, in agency vehicles, as needed.

Provide translation services, both written and oral, for shelter clients as needed.

Network with other service providers and collaborate with existing staff to assist clients with accessing community resources.

Plan and lead educational support groups for clients.

Assist supervisor in training staff and volunteers as needed.

Arranges for clients to meetings with law enforcement agencies, legal services, medical providers or other social service programs as needed.

Must be able to work flexible hours.

Minimum Requirements:
Bachelor’s degree (B.S.) in Social Work or related field required. Demonstrated proficiency of skills in crisis intervention, case management, social work theory and ethics. Experience in and knowledge of family violence issues preferred. Bilingual in Spanish/English a plus. Must have current Texas Driver’s License and automobile liability insurance.
