Jobing Description
Liaison with and making referrals to other agencies
  • Participating in multidisciplinary teams and meetings
  • Maintaining accurate records and preparing reports
  • Participating in training, -in-servicing and team meetings
  • Scheduling resident transportation for medical, dental, and vision appointments
  • Attend and participate and resident Care Plan Meetings
  • Follow-up with facility grievances and assist with solutions
  • Work closely with hospice companies and educate families on hospice
  • Participates with resident admissions
  • Participates on the Admissions team
  • Conducts room rounds
  • Works closely with Nursing coordinating patient care
  • Participates in the referrals to other agencies
  • Assists Administrator with discharge notices

    • Skills / Requirements
  • Possible Weekend duty
  • Must be a licensed social worker
