Field RN Case Managers are professionals that provide specialized cardiovascular and orthopedic care in a home based setting.

Field RN Case Manager treat patients suffering from CHF, COPD or are in need of anticoagulation therapy due to total hip/knee replacements. Average visits require the nurse to run assessments, provide teaching, manage anticoagulation, and treat CHF/COPD as indicated.

Our nurses must also act as a professional liaison between the doctors, the agency and other disciplines.

Licensed to practice professional nursing in Texas as an RN
ICU / CCU / Telemetry / ER or cardiac care related experience preferred.
Good people skills and ability to communicate with various levels of people
Ability to work in varied environments
Ability to self-manage with follow-through to complete tasks
Ability to work with others as well
Detail-oriented with ability to assess facts and draw conclusions
Good with systems and procedures
A good comfort level with computers is a plus
