Jobing Description
The On-Call RN CASE MANAGER is an integral part of the hospice interdisciplinary care team and as such is responsible for delivering nursing services to the hospice patient and family, as directed by the Director of Nursing Care and the Plan of Care. Understanding that hospice care must be delivered according to accepted standards of practice, guided by the mission of the organization and the patient/family's needs and concerns, the RN CASE MANAGER blends all services of the hospice into an integrated program of support.


This is considered a local, traveling position, requiring the employee to travel to and from patients homes, nursing facilities, hospitals, etc. throughout the Dallas / Fort Worth Metro Area.

Relationships:

  • Reports to the Director of Nursing Care.

  • Works with the entire interdisciplinary team and volunteers

  • Liaisons with community referrals, agencies, and health care professionals and families to provide greater understanding of hospice care and philosophy.

  • General number of patients assigned: 12 - 15


    Duties and Responsibilities

  • Provides hospice admission information to the patient and family referral sources as directed.

  • Participates in developing the interdisciplinary team plan of care.

  • Provides patient care services as directed by the plan of care.

  • Teaches appropriate palliative interventions to family members, other health professionals and the community at large.

  • Implements patient care services as directed by the plan of care.

  • Reports changes in the patient's condition to the Director of Nursing Care or designee, physician, or interdisciplinary team as appropriate.

  • Participates as directed by the bereavement plan of care in providing bereavement services.

  • Provides accurate, timely, appropriate documentation of all services provided to be placed in the Hospice Service Record within seven (7) days.

  • Attends team meetings and in-services as required.

  • Participates in presenting hospice related information to volunteers, other health care organizations, and the community at large.

  • Uses appropriate communication skills and provides emotional support to the patient/family as needed through good listening skills.

  • Contributes to program development through building relationships with potential referral sources in the area assigned.

  • Follows all appropriate policies and procedures of Lion Hospice. Follows all directives of practice act governing RN Case Managers.

  • All other duties as assigned.

  • Skills / Requirements
    Knowledge and Abilities:
  • Ability to develop good rapport with patients/families, team members and other health care professionals.

  • Ability to deliver care as directed by the plan of care and the Director of Nursing Care.

  • Sound knowledge of pain and symptom management.

  • Knowledge of the nursing process and accepted standards of nursing practice.

  • Knowledge of the nurse practice act.

  • Ability to exercise good judgment.

  • Excellent communication skills

  • Knowledge of the hospice philosophy and concept of care.

  • Ability to respond appropriately in crisis situations.

  • Ability to document accurately the delivery of hospice services.

  • Important Notes
    Qualifications:

  • Current and valid state Nursing License.

  • Reliable personal transportation for travel to and from patients and facilities.

  • Current and valid state driver's license and current auto liability insurance.

  • Training in disease processes leading to death (will be provided).

  • Training in control of pain and other symptoms (will be provided).

  • Training in infection control and universal precautions (will be provided).
