Job: Chaplain

Summary:
The Chaplain is an integral part of the interdisciplinary care team and as such is responsible for assessing the spiritual needs of the hospice patient and family. Understanding that spirtual servies must be delivered in a manner consistent with patient/family beliefs and desire for service, the Chaplain coordinates spritual services offered though the local hospice staff, local religious organizations, or community clergy.

Duties:
Evaluates referrals to assure appropriate admissions
Implements hospice information visits and sign up visits
Participates in plan of care with IDT 5 within 5 days of admissions
Coordinates spirtual needs upon admissions
Involves patient/family religious representives
Makes patient visits
Documents all visits, phone calls, and places documentations in clinical record within 7 days
Serves as a member of the IDT
Serves as spirtual resource to staff and volunteers
Assist clients in after death arrangements, including assisting with legal paperwork
Coordinates and Participates in presenting hospice programs for all
Other duties as assigned

Relationship:
Works as a Liaison with community religious organizations bringing greater understanding of hospice philosphy.

Requirements:
Trained in spirtual care by an accepted religious organization
Clinical Pastoral Education or equivqlent counseling course desired
Ordained or credential in religious organization of choice
