Heritage Gardens Healthcare Center provides services for residents requiring Skilled Nursing Care and Long Term Care. Additionally, we offer respite and hospice care. We are certified by Medicare and Medicaid and accept private payment as well as most insurances.

Outpatient Rehabilitation Patients or Residents who are receiving Skilled Nursing care can continue their comprehensive therapy program after discharge home. With an order from the physician, Occupational, Physical and/or Speech Therapy can be continued with our therapy staff on an outpatient basis. Referrals for outpatient therapy are welcome from the community as well.
