Custom Care Home Health & Hospice is seeking an experienced Hospice Registered Nurse. This individual must be a certified, registered nurse with the Texas State Board of Nursing who has completed the appropriate training/competency.The ideal candidate is a dedicated patient advocate, compassionate, team-oriented, driven, creative, technologically adept, quality focused, and positive.
We are currently interviewing for full-time registered nurses to help us in the DFW Metroplex - Dallas and Fort Worth.

About the Opportunity
The Hospice R.N. provides professional nursing care to develop, implement, and evaluate the patient's plan of care. The Hospice R.N., as the case manager, provides direct patient care and works collaboratively with other care team members, the physician and the patient/family to achieve optimal patient outcomes.
Team members make decisions both independently and in collaboration with others. Case manager assists with on call assignments as well. The nurse will case manage in Tarrant County including Fort Worth, HEB area, NRH area etc.
* Bachelor of Science in Nursing preferred but not mandatory
* Current Texas nurse license, valid Driver's license, proof of Motor vehicle insurance required
* CPR certification required
* At least one year of experience in hospice
* Experience documenting within the HomeCare HomeBase (HCHB) system preferred.
* Must possess the ability to effectively manage time and complete documentation on a tablet into an electronic patient record
(Spanish speaking is desirable but not required)
Salary: Competitive Wages commensurate with experience
Shifts: Full-Time, On-call
